CARE & HOUSING · IN DEVELOPMENT

Care that sees
the whole person.

We are developing adult case management and housing support in California, with an initial focus on San Diego.

The next step is easier
with someone beside you.

Health, housing, and everyday stability are deeply connected. Our planned division will help adults navigate complex needs and connect with appropriate care and community resources.

Our focus includes adults facing homelessness, serious mental health or substance use challenges, and the transition back into the community after incarceration.

PLANNED SERVICES

Care. A home.
A stronger tomorrow.

Four ways we plan to help adults move toward greater stability, with thoughtful support at each step.

Care that connects

Enhanced Care Management

A planned, person-centered approach to helping eligible adults coordinate complex health and social needs.

Support would include understanding each person’s priorities, developing a shared care plan, and helping connect medical, behavioral health, and community services.

Our goal is to make the next step clearer, strengthen communication between providers, and reduce the burden of navigating care alone.

A path toward home

Housing transition navigation

Helping eligible adults understand their housing options and work through the steps toward a place to call home.

Planned support includes identifying barriers, organizing documentation, navigating applications, and connecting with housing resources and potential landlords.

The focus is practical guidance and an individualized path forward; housing availability will depend on local resources.

A supported beginning

Housing deposits

A security deposit or other eligible move-in expense can stand between someone and a stable home.

We plan to help eligible participants navigate assistance for approved, one-time housing costs through the appropriate program.

Covered expenses and available assistance will depend on program rules, eligibility, and authorization. This is not a promise of ongoing rent payments.

Stability that lasts

Tenancy & sustaining support

Keeping a home can require support long after moving day.

Planned services focus on helping eligible adults understand tenancy responsibilities, communicate with landlords, connect to community resources, and address challenges that could put their housing at risk.

Our goal is to help people build confidence, strengthen everyday stability, and sustain a home over time.
